HIPAA Marketing Authorization

By checking the box on the previous screen, you understand and agree to the use and disclosure of your personal data to send you relevant advertising on third-party platforms and/or about partner products and services that may be relevant to you.  You understand that, to do this, certain personal data – like full or hashed email address and phone number, identifiable device and network identifiers, and information collected about your use of Aeroflow web pages – may be shared with third-party vendors and platforms that support Aeroflow’s advertising and marketing activities.  You further understand that Aeroflow may receive payment or other remuneration from third parties for making or facilitating these communications or partnerships.

• Granting authorization is voluntary.  You will still be able to use Aeroflow if you do not agree to this authorization.
• Information disclosed pursuant to this authorization may be subject to re-disclosure and no longer protected by HIPAA in the hands of the recipient.  
• This authorization is valid until revoked or 1 year from the date signed, or such shorter period as required by state law, whichever occurs first.  
• You may revoke this authorization by notifying Aeroflow in writing at dataprivacyofficer@aeroflowinc.com.  Revocation will not affect any uses made by Aeroflow before your revocation in reliance on this authorization.  In addition, your granting or revocation of authorization will not affect Aeroflow’s use of other standard Internet technologies that collect information from your device and browser in connection with your use of the Aeroflow web pages, as described further in Aeroflow’s Privacy Policy, where permitted by law without an authorization.
• At any time, you can view a copy of this authorization online or request a copy of this authorization from Aeroflow in writing at dataprivacyofficer@aeroflowinc.com.
